Background: Lives in Columbus, OH with his dog Chance, only child. Parents live locally.
Works part time in Construction. Not currently partnered. No previous psychiatric history.
Symptoms began in the last 2 months when he discovered he is being activated with the Navy
Reserves. His MOS is CM3 Construction Mechanic; no medical illnesses, no legal hx. Allergies:
NKDA; sleeps 8hrs; appetite good. Diagnosing Patients with Anxiety Disorders 

Mr. Ralph is a 19-year-old who is been activated with the Navy and has decided to set up an appointment with a psychiatrist. Mr. Ralph with a sad , tearful, and anxious affect expressing his sadness and fear of coming back to active duty due to his sexual orientation. Mr. Ralph feel anxious about the situation in which the people in his unit arrive to know that he is gay. Mr. Ralph have been hiding his sexual orientation, and now he feels terrified about their comments about gay people. Mr. Ralph fear the change that will happened when they know about his sexuality , like the man will no longer hug him, sleep close to him or been in the shower. Mr. Ralph fear been treated differently like people stand up and go away from him. He is confuse about close friend and sexual feelings.


During the interview Mr. Ralph have learn the he have alack of skill , the first one is been able to tell people that his gay and a second one is not able to face or deal with rejection due to his sexual orientation. Mr. Ralph worries a lot about rejection and that sometime make him feel that he want to die. Mr. Ralph currently is not suicidal but have the fear that he not part of them or feel that he is on the outside. Mr. Ralph want to stop to hide his real sexuality and does know how to do that as the fear of treated differently is challenging.

• Reflection notes: What would you do differently with this client if you could conduct the session over? Also include in your reflection a discussion related to legal/ethical considerations (demonstrate critical thinking beyond confidentiality and consent for treatment!), health promotion and disease prevention taking into consideration patient factors (such as age, ethnic group, etc.), PMH, and other risk factors (e.g., socioeconomic, cultural background, etc.).
